Your insurance helps pay for your visit and lab tests. We need your current insurance card to send the bill the right way.
That is okay. Tell our staff. We can talk with you about your options and see if you qualify for assistance by having you meet with our Case Management Team.
You may… Some lab tests are covered by insurance. Some are not. Every insurance plan is different.
You may have to pay if:
You have a deductible.
A deductible is the amount you pay for healthcare before your insurance starts paying its share.
You have a copay or coinsurance.
A copay is a fixed amount you pay for a healthcare service.
Coinsurance is the percentage of the cost you pay after you have met your deductible.
Your insurance does not cover the test.
Quest Diagnostics and LabCorp are separate companies. They send bills for the lab work they perform.
Your insurance decides how much they will pay.
Do not ignore the bill.
Quest Diagnostics and LabCorp may have patient assistance programs that may lower or remove your balance if you qualify.

An Explanation of Benefits (EOB) is not a bill.It is a letter from your insurance company that tells you:
What care or tests you had.
What your insurance paid.
What your insurance did not pay.
If you may still owe money.
The Explanation of Benefits helps you understand how your insurance used your benefits.
Remember: An EOB is for your information only. It does not mean you need to pay right away. If you owe money, you will get a bill from your doctor's office or the lab.

Central Outreach cannot pay lab bills from Quest Diagnostics or LabCorp.
If you receive a bill, we encourage you to contact your insurance to understand why they did not cover the lab charges, and email us as soon as possible and apply for assistance if you qualify.
If the remaining balance is your responsibility, you can pay your Quest Diagnostics bill online, or by clicking http://www.questdiagnostics.com/bill to pay your balance.
Call the phone number on your bill first. You will also need your insurance card to verify your identity with the insurance company.
